Sure, it is a failure from the perspective of public interest. It isn’t an accident.
I dunno, man. From where I sit, our governments look like systems designed to facilitate the extraction and exploitation of value from natural and human resources for the benefit of capital enterprise. From THAT perspective, it’s neither a failure nor an accident.
